The man was injured in the motorbike incident near Comboyne - 67 kilometres south west of Port Macquarie - just before lunch on Sunday.
A spokeswoman for Ambulance NSW said paramedics were called to a property at Pfeiffers Road, Innes View, about 1.30pm.
The ambulance arrived at 2.15pm and found a 66-year-old man who had suffered an injury.
"He had fallen from the quad-bike and the bike had rolled onto the patient," the spokeswoman said.
The man had left leg and right ankle injuries which were so severe the rescue helicopter was called.
It arrived at the property about 2.50pm and paramedics loaded the man on board.
He was transferred in a serious but stable condition.
The Ambulance NSW spokeswoman said the patient had a possibly fractured leg.
He arrived at Port Macquarie Base Hospital about 3.20pm.
A NSW Health spokeswoman said the man was in stable condition on Monday morning.
